We did you get this text from? It reminds me of typical email scam. If this is actually something you received by email, triple check that you know the sender and don't blindly complete the form.

It means that something was ordered. And for some reason, it was canceled. The form is about reimbursing what was already paid and canceling the rest of the payment.
